All patient registrations are now completed online. Please speak to reception if you think you will have difficulty with this.

Please note we can only accept patients who reside within the practice boundary shown. Use our postcode checker below to check that you live within our catchment area.

Patients are registered with the practice as a whole and not with a named doctor. 

As it often takes some time for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire.

Please note that if you do not submit all of the information within 24 hours the registration will reset and you will need to start again.

Proxy Access

This is access you can request for another patient, subject to passing identity checks and their written permission. It may enable you to book appointments, order repeat medications, see their immunisation history and test results/investigations.
