Your patient record is held securely on the electronic system at this practice. If you need treatment in another NHS setting — such as an Emergency Department or Minor Injuries Unit — the Summary Care Record allows those treating you to access key information from your GP record.

What is the Summary Care Record?

The Summary Care Record (SCR) is used nationally across England. It allows authorised NHS staff to view a summary of your GP record when treating you away from your usual practice.

Your permission will be asked before the information is accessed, unless the clinician is unable to ask you and there is a clinical reason for access.

Who can access it

Authorised healthcare professionals across all NHS settings — including urgent care, community care and outpatient departments in England.

What information it contains

  • Your current medications
  • Any allergies you have
  • Any bad reactions you have had to medicines
  • Additional information (if you have requested this is included)

Opting out

If you would like to opt out of the Summary Care Record, please ask at reception for our opt-out form.

A parent or guardian may request to opt out children under 16, but it is ultimately the GP's decision whether to create the records because of their duty of care to the child.
