Registering as a patient - You will need to either call into the surgery to complete forms or download the forms below to register with us here at Brunswick Surgery. Registrations are usually completed within 3 days of receipt.

Under the contract, patients are registered with the practice as a whole and not with a named doctor. Nevertheless, you do have the right to express a preference as to which GP you would like to see and this should be stipulated on the Patient Registration Form. We will endeavour to ensure that you always see your preferred GP, but trust you will appreciate that this will not always be possible.
